How to fill in the SpTH or Spanish sanitary form


If you are going to travel within the national territory, please consult the information provided by the Autonomous Communities regarding their requirements to the traveler by means of the following

The health control form (FCS) is a questionnaire about the health and travel conditions of the person individually or his/her family group, which should be done 48 hours before the trip, there are two types of tests: PCR polymerase chain reaction (nucleic acid) or antigen, plus the complete vaccination schedule of the Sars Cov2 whose authorized vaccines are: AstraZeneca, Janssen (Johnson & Johnson), Moderna, Nuvaxoid, Pfizer-BioNTech, Covishield, Sinopharm and Sinovac.

You must present this document at the health control at the destination airport. You can print it, or show it from your cell phone. This document is personal and non-transferable. You are responsible for the information provided, which is part of this document or QR, as well as for its safekeeping.

Código QR Spain Travel Health (Spth) recibido en .pdf por emailUna vez correctamente rellenado, se genera un código QR que debemos presentar en la pantalla de nuestro móvil cuando nos sea requerido, generalmente en nuestro aeropuerto de origen pero siempre en un control específico en nuestro aeropuerto de llegada.

El Código QR también lo puedes recibir en .pdf para imprimir y en formato .pkpass para tener en tu móvil en la aplicación monedero que uses para guardar tarjetas de embarque, entradas de teatro, etc.

RECUERDE que para viajar a España debe traer el documento/certificado acreditativo indicado o facilitado al rellenar el formulario. Los menores de 12 años no están obligados a presentar este certificado. Es posible que se realice una prueba diagnóstica a la entrada en España.

Deberá presentar este documento en el control sanitario del aeropuerto de destino. Puede imprimirlo o mostrarlo desde su móvil. Este documento es personal e intransferible. Usted es responsable de la información proporcionada, que se incluye en este documento o QR. También es responsable de su custodia. Recomendamos su destrucción/borrado después de haberlo mostrado en el control sanitario del aeropuerto.
